Pizhichil ayurvedic treatment is a warm oil therapy where medicated oil is poured in a slow, unbroken stream over your body while two to four therapists massage in rhythm. In Ayurvedic terms it brings together Snehana, which is oleation, and Swedana, which is sudation. Many Kerala families call it Thailadhara. It is one of the older classical therapies still practised in the same way, and it is usually chosen for stiffness, joint discomfort, and nerve-related complaints rather than for a quick spa hour.
A session runs 45 to 60 minutes. Most guests are advised a course of 7 to 14 days, because the therapy is understood in Ayurveda to work cumulatively rather than in a single sitting. Your physician decides the actual number after examining your constitution and current condition.
At our resort in Kovalam, nobody starts oil on your skin before a consultation. Vaidyar J. Christudas examines you, notes your prakriti and the dosha imbalance he finds, and selects the oil formula from there. The treatment plan you receive is not the same one the guest in the next room receives.
You lie on a wooden treatment table, the traditional droni. A cloth band is tied across your forehead. This keeps oil out of your eyes and, just as important, keeps your head from overheating. In classical practice the head is kept cool while the body is warmed.
Cotton cloths are dipped in lukewarm medicated oil and squeezed over your body in a steady stream. Therapists on either side follow the pour with long strokes, moving up and down the limbs and torso in time with each other. The head is left out of the pouring.
The therapy moves through seven standard positions, so no part of the body below the neck is missed. Oil is warmed fresh each day and never reused from a previous session. Everything happens with the physician available to adjust temperature, pressure, or oil choice partway through a course if your response suggests it.
The sensation surprises people. It is warm, heavy, and quiet. There is no deep pressure, no cracking, and no strain.
These are the effects described in Ayurvedic practice and reported by guests. Individual response varies, and Pizhichil is not a substitute for medical care you are already receiving.
Ayurveda has used Pizhichil for a defined group of vata-related complaints. Below is where it is most often advised. This is a description of traditional use and not a promise of outcome. If you are being treated for any of these, bring your reports and current medicines to the consultation so your plan can be built around them.
A note we would rather state plainly: Ayurveda describes Pizhichil as supportive therapy for these conditions. It does not cure them. Anyone who tells you a fortnight of oil therapy will cure arthritis or reverse paralysis is overselling it.
Guests often arrive having read about all three and unsure which to book. The short version is that they treat different things.
Pizhichil: Covers the whole body below the neck. Oil is poured continuously and worked in by several therapists at once. It combines oil with heat, and it is the heaviest of the three. Chosen for joints, muscles, and nerve complaints.
Shirodhara: Works only on the head. A thin stream of warm oil or another liquid falls on the center of the forehead for 30 to 60 minutes while you lie still. There is very little massage. Chosen for stress, insomnia, headache, and mental fatigue.
Abhyanga: A full-body oil massage with hands only. The oil is applied and worked in rather than poured in a stream, and there is no sustained heat component. Lighter, shorter, and often the entry point for first-time guests or for general wellness.
If you are unsure, book the consultation and let the physician decide. Quite often the answer is a combination: Abhyanga to prepare the body, Pizhichil for the main course, and Shirodhara in the evening for sleep. That sequencing is what our treatment packages are built around

Pizhichil is generally well tolerated when a physician supervises it. Mild, short-lived effects can include drowsiness, a feeling of heaviness, temporary soreness, or loose stools as the body responds to the oil. Skin irritation is possible if an oil ingredient does not suit you, which is why the formula is chosen after consultation. Report anything unusual to the physician the same day.
Eat something light two to three hours beforehand and avoid a heavy meal. Empty your bladder before you go in, wear or bring clothes you do not mind staining with oil, and skip alcohol the night before. Bring any medical reports and your list of current medicines.
Not straight away. Plan to rest for at least an hour after the session. Avoid a cold shower, strong air conditioning, cold drinks, and strenuous exercise for the rest of the day. A warm bath after the advised waiting time, light food, and an early night get you the most from the therapy.
Pizhichil is one of the therapies most often advised in Ayurveda for arthritic joint pain and stiffness, and guests commonly report easier movement across a course. It is supportive therapy rather than a cure, and it should sit alongside the treatment your doctor has prescribed, not replace it.
It can help with the dryness, scaling, and itching that come with psoriasis and eczema, because the medicated oils nourish the skin directly. Oils are selected for the skin condition in these cases. Anyone on dermatological treatment should continue it and mention it at consultation.
